Service counselling for the elderly

Service counselling for the elderly is a free service for the elderly and their immediate close persons.

The purpose of service counselling is to support the elderly living at home and to work together to find solutions for a smoother everyday life. The goal is to find the right kind of help at the right time.  

The social services for the aged, which are the responsibility of the wellbeing services county, can be accessed through service counselling. Services are always granted on the basis of an individual assessment of the need for services and in accordance with the criteria for granting services for the elderly as approved by the Päijät-Häme wellbeing services county executive. The assessment of the need for services may be requested by the client or, with the client’s consent, by a close person, a public authority, or another person familiar with the client’s situation.

When you need information about the services providing support in the everyday life of the aged, contact the service counselling for customer guidance for the elderly. You can contact us by visiting the service point of the service counselling in person, by calling, or by using the chat service.  

If the client’s situation requires a more extensive assessment of the need for services, a home visit by a client counsellor or a contact with the gerontological social services will be arranged for this purpose. 

Assessment of the need for services through a home visit by a client counsellor

During the service needs assessment visit, the client counsellor will analyse and assess the client’s need for support and service in a comprehensive fashion in collaboration with the client and, if necessary, their close persons. In course of the assessment of the need for services, the following topics will be taken into account from the client’s point of view:

  • Physical, mental, cognitive, and social capabilities via RAI assessment tools
  • The client’s own resources, social networks, and support provided by family members
  • Home safety, accessibility of the living environment, and access to local services
  • Social benefits

Based on the assessment, services tailored to individual needs are planned out for the client. The starting point for the provision of services is the client’s own view of their situation and the choices they make in order to solve it.
